Phase one of the United States Medical Licensing Examination® (USMLE®) Service Transition is now complete. All USMLE Step exam services for students and graduates of international medical schools (IMGs), including exam registration, score report delivery, and USMLE customer service, are now managed by USMLE co-sponsor the Federation of State Medical Boards (FSMB) through its USMLE portal.

Important Information About ECFMG Certification
ECFMG®, a division of Intealth®, continues to manage the certification process, and IMGs should continue to contact ECFMG with any questions related to ECFMG Certification.
IMGs still use MyIntealth™ to:
- Request a MyIntealth account;
- Submit the Application for ECFMG Certification;
- Submit medical education credentials for primary-source verification;
- Monitor the status of their applications, cases, and progress toward meeting the requirements for ECFMG Certification; and
- Access other certification-related services.
There are no changes to the requirements for ECFMG Certification as a result of the USMLE Service Transition. ECFMG continues to require IMGs to pass USMLE Step 1 and Step 2 Clinical Knowledge (CK) to meet the medical science examination requirement for ECFMG Certification.
ECFMG continues to determine whether IMGs are eligible for ECFMG Certification, and eligibility for certification continues to be a core requirement for IMGs to take USMLE. This means that, as part of the USMLE registration process for each Step, FSMB will confirm with ECFMG that IMGs are eligible for ECFMG Certification. The USMLE program will share exam outcomes with ECFMG directly as scores become available.
An updated 2026 ECFMG Information Booklet is now available on the ECFMG website. All applicants pursuing ECFMG Certification must use the most current edition of the ECFMG Information Booklet available to them at the time of inquiry, and must become familiar with, and will be subject to, the policies and procedures detailed therein.
