Hospitals, HMOs, and other organizations that require confirmation of ECFMG Certification for employment purposes may submit those requests via CVS ON-LINE or by submitting a completed request form (Form 236A) to ECFMG. All requests must include the ECFMG-assigned organization number. The fee for this service is $35.00 per report.

Organizations that request reports using Form 236A will be billed monthly for the previous month’s activity. ECFMG accepts checks, money orders, and credit cards (Visa, MasterCard, Discover, and American Express) as payment for this service. A response will be sent via an express mail service if a prepaid airbill or express service account number is provided with the request form. Credit card payments must be accompanied by a completed credit card payment form (Form 236A2-CC).
